Support Worker - Assisted Discharge. Location: Royal Bournemouth Hospital and Poole Hospital. Salary: £24,479 per annum, pro rata (£13.45 per hour). Hours: 21 per week including weekends and Bank Holidays. Contract: Permanent. Driving: Full UK Manual Driving Licence required, held for a minimum of 2 years. Please note: this is a 7 day, 365 day service and you will be working weekends and Bank Holidays at both Royal Bournemouth Hospital and Poole Hospital.

Whether it's helping with shopping, checking in with a phone call, or simply being a friendly face, your support will help people live more independently.
What will a day in the life of a Support Worker involve?
* Providing practical and emotional support, including welfare calls, shopping, and home visits.
* Supporting adults after a hospital stay to help them recover safely at home.
* Responding to referrals from clinicians and community health professionals.
* Assessing needs and following a personalised support plan for each service user.
To be a successful Support Worker, what will you need?
* A kind, calm and confident approach with great people skills.
* The ability to work independently and as part of a team.
* A full UK driving licence and willingness to drive service vehicles.
* Basic IT skills and a commitment to following procedures.
